I was surprised and very saddened when I learned of the David's death
much after the fact, and I am still curious about exactly what happened.

In any case I can explain the manuscript and correspondence you found. I
was project director on an NSF grant for COMAP to develop materials for
high school students by distingished research mathematicans.  David
agreed to write a module about box packings for the series. He started
work and if I recall sent me a first draft of the first chapter (I am
not sure where that is now, or if I even still have it) but at some
point a believe wrote me that he would be unable to finish or do anymore
work. At that time, and even now, there is no unified treatment that I
know of at a "low" level about box packings.

Is it your plan to put these unfinished materials up on the web? Can you
send me a photocopy of what's there?
